CONSTITUTION OF THE DOWNLAND PRACTICE PATIENT REPRESENTATION GROUP Name: The Group will be called The Downland Practice Patient Representation Group Members: The Group will consist of representatives from each Parish Council served by the Practice, and a representative from the Downland Volunteer Group. Other representative bodies of patients may be members, if so decided by the Group. Each member will be nominated by their Parish Council to represent that Parish Council. If it is felt by the Practice that any one member of the Group is not truly representing the views of the Parish Council, then the Practice may ask the Parish Council to nominate another member. Objectives: The purpose of the group is:To act as a forum for the two-way flow of information between
patients and the practice, the representative reporting such information back to
the body which has nominated them.To discuss ideas on the day to day delivery of services
provided by the practice, as a result of which changes to procedures may be made
by the practice.No individual concerns that may identify a patient will be
discussed, only problems relating to systems of service delivery. Meetings will be held quarterly, at which minutes will be taken and circulated to all members.
